One refresh is available per quarter and you pay only when you use one. Nothing bills for a quarter you did not use, and nothing recurs. The quarters run from your purchase date, the same clock as the renewal.
$500 rebuild: new pages, new sections, navigation changes, or a restructure. That is a new build, so it is priced as one. It does not reset your hosting year: the renewal date stays anchored to your original purchase.
